Executive Summary:
PATH CITED (Capacity and Infrastructure Transition, Expansion and Development) is intended to support the delivery of Enhanced Care Management and Community Supports services, specifically addressing capacity needs to support the transition, expansion, and development of processes to prepare Health Services to contract with our local Medi-Cal managed care plan (Partnership HealthPlan), to better serve Medi-Cal members under CalAIM.
