Subject: Consideration of patch payment on a Public Guardian conservatee “BB” from general funds in an amount not to exceed $30,000 at $5,000 per month.
Executive Summary:
We respectfully request BOS approval of a patch payment for BB, a conserved individual under our care as Public Guardian, for the County of Lake, at the recommendation of our Deputy Public Guardian, Lisa Walker-Coleman.
BB requires placement at Nazareth Classic Care to ensure appropriate care and support for his unique needs. Due to the level of care required a patch payment is necessary to cover the additional cost beyond the standard rate.
Public Guardian has conducted an assessment, and without this additional funding from the general fund, it will be nearly impossible to secure a stable and safe placement for BB. The patch payment will allow him to be placed in a facility that is well-suited for his medical and
psychosocial needs, promoting both his well-being and stability.
Attached, you will find detailed documentation supporting this request, including an analysis of
BB’s current condition, the required level of care, and the cost breakdown for the placement. We have also attached two examples of similar historical patch payments for “TL” and “AB”.
We appreciate your time and consideration in this matter. I am confident that this patch payment will significantly improve the quality of life for BB and help ensure his continued well-being under conservatorship.
