The County of Lake entered into an agreement with Willow Glen Care Center effective July 1, 2025, for Adult Residential Support Services and Specialty Mental Health Services for Fiscal Year 2025-2026. The original agreement amount was $500,000.00.
Since implementation of the agreement, utilization of services has exceeded initial projections, reflecting increased demand for Residential and Specialty Mental Health Services within Lake County. Amendment No. 1 increases the total compensation payable under the agreement by $500,000.00, resulting in a revised contract maximum of $1,000,000.00, and amends Section 3 (Compensation) to reflect this change.
Historical expenditures for Willow Glen services have steadily increased over the past three fiscal years, reflecting both increased utilization and higher placement costs:
* FY 2023-2024: $566,694.26
* FY 2024-2025: $710,554.41
* FY 2025-2026: $713,596.64 invoiced to date
During FY 2025-2026, Willow Glen programs have served approximately 10 clients to date across multiple levels of care, including Mental Health Rehabilitation Centers (MHRCs), Intensive Residential Care (IRC), Adult Residential Programs, and Residential Care Facilities for the Elderly (RCFE). Current utilization averages approximately 259.6 bed days per month. Increased expenditures are attributable to both higher placement utilization and provider rate increases between FY 2024-2025 and FY 2025-2026.
The Willow Glen system includes Sequoia Center MHRC, Cedar Grove MHRC, Willow Glen Intensive Residential Care/Golden Beginnings, Rosewood Adult Residential Program, and Redwood Creek Residential Care Facility for the Elderly. Through these programs, the County has served individuals with significant and persistent mental health conditions, including co-occurring disorders, cognitive impairments, forensic involvement, medical complexity, and long-term residential treatment needs.
Over the past three fiscal years:
* Sequoia Center served 6 unique individuals, with 1 long-term placement exceeding 12 months.
* Cedar Grove served 4 unique individuals, with 1 placement exceeding 12 months.
* Willow Glen IRC/Golden Beginnings served 5 unique individuals, with 1 placement exceeding 12 months.
* Rosewood served 4 unique individuals with no placements exceeding 12 months.
* Redwood Creek served 7 unique individuals, including 2 long-term placements exceeding 12 months.
Several clients successfully transitioned to lower levels of care during this period, demonstrating the effectiveness of step-down treatment planning and ongoing discharge coordination. Examples include transitions from locked MHRC placements to Board and Care settings, reunification with family supports, and movement to community-based placements with third-party assistance.
Current placements within the Willow Glen system reflect varying levels of acuity and treatment need:
* Sequoia Center currently serves one client admitted from Lake County Jail who is demonstrating gradual clinical progress following a period of self-isolation, with discharge planning underway for possible housing step-down placement.
* Cedar Grove currently serves one client transitioned from another locked MHRC to access increased programming, activities, and community outings while continuing to require structured treatment support.
* Willow Glen IRC/Golden Beginnings currently serves one client with longstanding lack of insight into mental illness who remains appropriately placed in a highly structured residential setting.
* Rosewood currently serves two clients who have successfully stepped down from higher levels of care, including one individual transitioning from jail and locked residential treatment into a lower level residential placement.
* Redwood Creek currently serves three clients with ongoing psychiatric and medical support needs who are benefiting from increased family proximity, structured residential support, and continued psychiatric stabilization.
Behavioral Health staff continue to monitor ongoing placement appropriateness, medical necessity, treatment needs, discharge readiness, and opportunities to transition clients to lower levels of care whenever clinically appropriate and feasible. Placement reviews and discharge planning efforts are conducted on an ongoing basis to support client stabilization, promote the least restrictive level of care, and ensure effective utilization of Specialty Mental Health Services resources.
The Contractor will continue to provide services in accordance with Exhibit A (Scope of Services), and payment will continue to be made in accordance with Exhibit B (Fiscal Provisions), contingent upon the Contractor not being in default under the agreement.
The agreement remains subject to any restrictions, limitations, or conditions imposed by County, state, or federal funding sources and is contingent upon the availability of sufficient funding. Funding for these services is provided through Behavioral Health program revenues and Realignment funding sources. No portion of this contract is Medi-Cal reimbursable, and there is no additional County General Fund contribution, local match requirement, or Intergovernmental Transfer (IGT) exposure associated with this agreement.
Except as specifically amended, all other terms and conditions of the original agreement remain unchanged and in full force and effect. This amendment ensures continuity of critical Specialty Mental Health Services, supports increased service demand, and allows the County to maintain compliance with applicable state and federal funding requirements.
