SUBJECT: (a) Approve the amended Facility Agreement between the County of Lake and Chapel of the Lakes for FY 16-17, retroactive to July 1, 2017, and authorize the Chair to sign; (b) Approve the amended Agreement between the County of Lake and Chapel of the Lakes for FY 16-17, retroactive to July 1, 2017, and authorize the Chair to sign; (c) Approve amended Agreement between the County of Lake and Jones Mortuary for FY 16-17, retroactive to July 1, 2017 and authorize the Chair to sign; and (d) Approve amended Agreement between the County of Lake and Jones and Lewis Mortuary for mortuary services for FY 16-17, retroactive to July 1, 2017 and authorize the Chair to sign.
EXECUTIVE SUMMARY: On June 28, 2016, Your Board approved Mortuary agreements with each of these Mortuaries. It was determined by the Coroner’s Division that there was some language that needed to be clarified. Those changes have been made and the revised contracts are ready for signature. No adjustments to costs have been made.
The Department will continue to utilize each of the three Mortuaries, on a rotating basis, as has been the procedure in the past. These agreements are attached for your review and approval. These agreements will be retro to July 1, 2016.

FISCAL IMPACT (Narrative): Funds are already budgeted in the Sheriff/Coroner budget 2201/23.80 for these costs. No additional funds are being requested at this time.
