Subject:                     Consideration of the Agreement Between County of Lake and Sierra Pathways, A Licensed Social Worker Corporation in the Amount of $960,281.57 for Fiscal Years 2025-2027.

 

Executive Summary:

Pursuant to a competitive procurement process conducted under RFP No. 25-1138, the Behavioral Health Services Department recommends approval of a contract with Sierra Pathways, A Licensed Clinical Social Worker Corporation, to implement and operate two critical behavioral health programs: a Day Treatment Intensive (DTI) Half-Day Program and an Adult Residential Facility (ARF), both designed to serve Transitional Age Youth (TAY) and adults with co-occurring mental health and substance use disorders, including those impacted by opioid misuse.

This project is consistent with the County’s Opioid Settlement Funds Expenditure Plan and aligns with the California Department of Health Care Services’ High Impact Abatement Activities (HIAA) framework. The proposed services will fill long-standing gaps in Lake County’s behavioral health system by offering evidence-based, culturally responsive care in both outpatient and residential settings. These services are particularly responsive to the needs of vulnerable populations, including individuals involved in the justice system, those at risk of homelessness, and historically underserved communities.

The awarded contract amount of $960,281.57 will be administered on a reimbursement basis over two years and will fund program startup and operational costs. After start up, this project will sustain through Specialty Mental Health Medi-Cal reimbursements.

Performance metrics and reporting requirements are embedded in the contract to ensure accountability, including quarterly outcome reports, financial documentation, and evaluation of opioid-related impacts.
