Subject: Approve Agreement for Physical Therapy Services in Support of the California Children’s Services Medical Therapy Program (MTP) Between County of Lake and Trina Maia for a Term of Twelve Months for an Amount not to Exceed $114,840 for the Term of the Agreement and Authorize the Chair of the Board of Supervisors to Sign
Executive Summary:
In an effort to provide Physical Therapy services to children in the California Children Service (CCS) program who are referred to the Medical Therapy Unit (MTU), Lake County Health Services (LCHS) recently completed a Request for Proposals (RFP) for Physical Therapist services.
LCHS received a proposal from Trina Maia, MPT in response to the RFP. With over 20 years of experience as a pediatric physical therapist, working with children from birth to 21 years of age, Trina Maia, MPT would provide the level of Physical Therapy services that LCHS is seeking. Ms. Maia was a Senior Physical Therapist with LCHS CSS for 8 years and we are fortunate enough to have the opportunity to bring her back and continue her service to the community.
LCHS respectfully requests that your board approve this contract to provide Physical Therapy services.
