Subject:                     Approve the Second Amendment to the Agreement between the County of Lake and June Wilson-Clarkin, LMFT for Fiscal Year 2024-2025 to (1) authorize the Contractor to initiate 5150 psychiatric holds upon designation and completion of required training, and (2) increase the contract maximum by $25,000 for a revised total of $110,000, and authorize the Chair to sign.

 

Executive Summary: 

 

Approval is requested for the second amendment to the Agreement between the County of Lake and June Wilson-Clarkin, LMFT, for Fiscal Year 2024-2025. This amendment includes two key modifications. First, Exhibit A - Scope of Services is amended to add Section 4.7, “Authorization for 5150 Hold Writing by Contracted LCSW,” which authorizes the Contractor-upon formal designation by the LCBHS Behavioral Health Director and successful completion of all required training-to initiate 5150 psychiatric holds under limited and defined conditions. This provision enhances the County’s ability to respond to emergency psychiatric needs when traditional evaluation pathways are not available.

 

Second, Exhibit B - Fiscal Provisions is amended to increase the contract maximum by $25,000, bringing the total contract amount to $110,000 for FY 2024-2025. This increase supports the additional responsibilities outlined in the amended scope of services. The Contractor’s authorization to initiate 5150 holds is contingent upon compliance with all applicable laws, departmental policies, and training requirements, and may be rescinded by LCBHS at any time. This amendment supports continuity of care while expanding the County’s capacity to address psychiatric emergencies in the community.
