Subject:                     Consideration of the Agreement Between County of Lake and Hilltop Recovery Services for ASAM Residential Level 3.1 & 3.5 and Intensive and Outpatient Levels of Care 1.0 & 2.1 in the Amount of $2,000,000 for Fiscal Year 2025-26

 

Executive Summary:

 

The County of Lake is entering into an agreement with Hilltop Recovery Services to provide a comprehensive continuum of substance use disorder (SUD) treatment services for Medi-Cal beneficiaries. Under this contract, Hilltop will deliver residential treatment at American Society of Addiction Medicine (ASAM) Levels 3.1 and 3.5, intensive outpatient treatment (ASAM Level 2.1), and outpatient services (ASAM Level 1.0). Services will be provided at Hilltop’s licensed facilities in Clearlake Oaks and Lucerne, California.

 

The agreement covers the period July 1, 2025, through June 30, 2026, with a total contract amount not to exceed $2,000,000. All services must comply with applicable state and federal regulations, including Medi-Cal requirements, the DMC-ODS waiver, HIPAA, and DHCS guidelines. Hilltop is responsible for maintaining appropriate licensure, ensuring medically necessary care, and adhering to documentation, quality assurance, and reporting standards.

Hilltop Recovery Services has been a cornerstone of SUD treatment in Lake County for many years, consistently demonstrating a deep commitment to serving our community with integrity and compassion. Their services play a critical role in ensuring that Medi-Cal beneficiaries have timely access to a full spectrum of evidence-based treatment options aligned with the Drug Medi-Cal Organized Delivery System (DMC-ODS) requirements.

Utilization data from FY 2024-2025 illustrates the significant role Hilltop continues to play in meeting the behavioral health needs of our residents:

                     ASAM Level 3.5 Residential Treatment: Delivered 2,766 units of Residential DMC services, 1,024 units of Targeted Case Management, and 756 non-billable support services.

                     ASAM Level 3.1 Residential Treatment: Delivered 224 Residential DMC services, 66 units of Targeted Case Management, and 61 non-billable services.

                     ASAM Levels 2.1 and 1.0 (IOT/ODF): Provided 1,142 units of Group Counseling, 247 units of Targeted Case Management, 197 units of Individual Counseling, and conducted 66 clinical assessments.

This partnership ensures that clients receive integrated, coordinated care that aligns with both state and federal guidelines, including medical necessity criteria, EPSDT requirements for youth, and access standards outlined in BHIN 24-001. Hilltop’s demonstrated capacity to deliver high-quality, culturally responsive care, including services to justice-involved and high-acuity populations, further reinforces the importance of sustaining this contract.

We sincerely appreciate Hilltop’s dedication to Lake County and recommend approval of this contract to continue this vital work.
