Executive Summary:
Lake County Health Services requests approval to accept $100,000 in funding from Partnership HealthPlan of California to be utilized over three years to support implementation of the Community Health Assessment and Community Health Improvement Plan Process.
This funding will enable the department to meet new California Department of Public Health regulatory changes and timeline requirements for Local Health Jurisdictions.
