Subject:                     Consideration of the Agreement Between County of Lake and California Psychiatric Transitions, Inc. for Inpatient Psychiatric Rehabilitation Services, Inpatient Competency Restoration Services, and Professional Services Associated with Inpatient Psychiatric Rehabilitation in the amount of $350,000 for Fiscal Year 2025-26.

 

Executive Summary:

Lake County Behavioral Health Services (LCBHS) seeks approval of the FY 2025-2026 agreement with California Psychiatric Transitions, Inc. (CPT), a licensed Institute for Mental Disease (IMD) located in Merced County, for the provision of high-level mental health rehabilitation services to adult clients with serious mental illness. This contract will not exceed $350,000.

CPT offers a secure, structured environment for individuals who require intensive residential support and are stepping down from acute psychiatric hospitalization or are otherwise unable to be safely managed in less restrictive settings. The facility provides services aligned with Title 9 regulations and California Welfare and Institutions Code Section 5600, offering individualized treatment that includes medication support, group therapy, individual counseling, and life skills development.

In FY 2024-2025, LCBHS placed two unduplicated clients at CPT. While overall utilization remains low, access to this level of care is essential for a small subset of high-acuity clients-particularly those who would otherwise require placement at a State Hospital due to the severity of their symptoms, behavioral instability, or forensic involvement. CPT is one of the very few facilities in California equipped and willing to accept such clients, offering a critical alternative to State Hospital commitment.

This agreement is consistent with the County Purchasing Ordinance, as CPT provides a specialized service not readily available from other vendors. LCBHS has contracted with CPT for several consecutive years due to the unique nature of the services provided and the provider’s ability to meet the clinical, legal, and safety needs of this highly vulnerable population. The continuation of this contract allows the County to fulfill its obligations under the Lanterman-Petris-Short (LPS) Act and Medi-Cal medical necessity criteria for adults with serious mental illness requiring IMD-level care.

CPT continues to meet regulatory requirements and provides timely documentation and coordination with County staff. Approval of this contract supports the County’s mandate to provide medically necessary mental health services to eligible beneficiaries and ensures timely access to high-level placements when clinically indicated.
