Subject:                     ADDENDUM - Adopt Resolution Approving Receipt of a Grant in the amount of $41,500 from Department of Health Care Services by the Probation Department for implementation of CalAim Enhanced Care Management

 

Executive Summary:

 

I am recommending your Board Adopt the attached Resolution approving receipt of a Grant in the amount of $42,000 from the Department of Health Care Services by the Probation Department for implementation of CalAim Enhanced Care Management.

 

In January 2024, the Probation Department applied for this grant to in order to begin Enhanced Care Management for those individuals reentering society from incarceration. On September 16, Probation was notified that it was preliminarily awarded the grant, and the final notification took place on October 16, 2024.  The grant provides the Probation Department with funding to purchase the necessary equipment, recruit employees, and train staff in the implementation of enhanced care management. With this grant funding, hire staff necessary for implementation, train staff, and probation staff will be able to assist those reentering society and on probation in the community.

 

Probation is required to match one half of the grant ($21,500) for this implementation which can be done with the current budget resources.
