Subject: Approve Amendment No.1 to the FY 2021-22 Agreement Between County of Lake and Ever Well Health Systems for a New Contract Maximum of $101,700.00 and Authorize the Board Chair to Sign.
Executive Summary:
Lake County Behavioral Health Services is contracted with Ever Well Health Systems for bed days in a Residential Crisis Facility which provides intensive residential services to people with severe mental illness. This facility provide 24/7 care and can support individuals with high-level, co-occurring medical needs. LCBHS has had one client placed there since 09/01/2020, which is 547 days at the time of writing this memorandum.
This placement is exactly what this client needs. In early 2020, LCBHS received notice from Davis Guest Home, a Board and Care facility, of their intent to discharge the client because they had become more medically complex than could be managed. Our team worked for months trying to place the client into a more medically appropriate setting. Eventually, the client was sent out of Davis Guest Home for medical reasons to Doctor’s Medical Center, an acute care facility. The client remained there from 6/27/20 - 9/1/20. Doctor’s Medical Center and LCBHS worked on trying to find an appropriate placement for the client. Denial after denial would come in. Thankfully, Ever Well has been able to meet both the client’s physical and mental health needs, including dialysis treatment 3x\weekly.
This contract Amendment increases the contract by $26,000.
