Subject: (a) Consideration of Contract Change Order No. 1 to the Construction Contract Between Lake County and Greg Simpson Trucking, Inc. for the Construction of the Socrates Mine Road Landslide Repair Project (Federal Project No. FEMA-4308-DR-CA) in the amount of $9,722.10, increasing the original contract amount of $491,864.53 to a new contract amount of $501,586.63 and Authorize the Chair to Sign the Change Order; and (b) Consideration of Contract Change Order No. 2 to the Construction Contract Between Lake County and Greg Simpson Trucking, Inc. for the Construction of the Socrates Mine Road Landslide Repair Project (Federal Project No. FEMA-4308-DR-CA) in the amount of $139,200.25, increasing the contract amount from $501,586.63 to a new contract amount of $640,786.88 and Authorize the Chair to Sign the Change Order
Executive Summary:
The Socrates Mine Road Landslide Repair Project is currently under construction. Staff has prepared two construction contact change orders. The change orders require approval by the Lake County Board of Supervisors.
The Public Works Director recommends that the Board of Supervisors approve Contract Change Orders Nos. 1 and 2 that incorporate (a) the construction of a culvert headwall and (b) additional road repairs and grading into the contract. Contract Change Order Nos.1 and 2 increases the contract amount by $148,922.35, from the original contract amount of $491,864.53 to a revised contract amount of $640,786.88.
Contract Change Order No. 1
Contract Change Order No. 1 incorporates the construction of a culvert headwall to prevent / limit soil erosion at a culvert opening at MP.
Contract Change Order No. 1 increases the contract amount by $9,722.10, from the original contract amount of $491,864.53 to a new contract amount of $501,586.63.
The cost of the Contract Change Order will be paid from non-discretionary local road funds.
Contract Change Order No. 2
Contract Change Order No. 2 incorporates additional road reconstruction and grading to address roadway damage not shown on the plans.
Contract Change Order No. 2 increases the contract amount by $139,200.25, from a contract amount of $501,586.63 to a new contract amount of $640,786.88.
Contract Change Order No. 2 will be paid from Geyser Geothermal Mitigation Funds for Socrates Mine Road.

Recommended Action:
The Public Works Director recommends that the Board of Supervisors:
a. Approve Contract Change Order No. 1 to the Construction Contract Between Lake County and Greg Simpson Trucking, Inc. for the Construction of the Socrates Mine Road Landslide Repair Project (Federal Project No. FEMA-4308-DR-CA) in the amount of $9,722.10, increasing the original contract amount of $491,864.53 to a new contract amount of $501,586.63 and Authorize the Chair to Sign the Change Order.
b.
