SUBJECT: Approve Amendment 2 to the Agreement between the County of Lake and Crestwood Behavioral Health for Adult Residential Support Services and Specialty Mental Health Services for Fiscal Year 2017-18 and Authorize the Board Chair to sign the Amendment.
EXECUTIVE SUMMARY: Attached, for your approval, is Amendment 2 to the Agreement between the County of Lake and Crestwood Behavioral Health for Adult Residential Support Services and Specialty Mental Health Services for Fiscal Year 2017-18.
BACKGROUND AND DISCUSSION: Crestwood Behavioral Health has numerous facilities throughout the State of California and serves multiple Lake County Medi-Cal beneficiaries. Crestwood Behavioral Health facilities include Psychiatric Health Facilities (PHFs), Skilled Nursing Facilities (SNFs), Mental Health Rehabilitation Centers (MHRCs), Adult Residential Facilities (ARFs), Community Programs, and Residential Facilities for the Elderly where clients are offered a continuum of care and treatment from traditional psychiatric services to innovative recovery programs geared toward community reintegration.
As Lake County Behavioral Health (LCBH) has clients placed at numerous Crestwood facilities and as it is our goal to keep Lake County clients at the lowest level of care possible, it would require extensive work to find new placement for these clients.

Original Contract Amount: $500,000
First Amendment Increase: $450,000
Second Amendment Increase: $350,000
New Contract Amount: $1,300,000
FISCAL IMPACT (Narrative): LCBH is requesting approval of Amendment 2 to the Agreement with Crestwood Behavioral Health for FY 2017-18 for an increase of $350,000 for a new contract maximum of $1,300,000. The funds for this contract will be re-allocated from unassigned funds within the same levels of care.

RECOMMENDED ACTION: Approve Amendment 2 to the Agreement between the County of Lake and Crestwood Behavioral Health for Adult Residential Support and Specialty Mental Health Services for Fiscal Year 2017-18 for an increase of $350,000 for a new contract maximum of $1,300,000 and authorize the Board Chair to sign the Amendment.
