Subject: Approve request to apply for the California Department Public Health/California Home Visitation Program grant funding for Public Health's Home Visitation Program in the amount of $470,000 annually for the FY 23-24 thru 27-28 and authorize the Department Head to sign the Affiliation Application

Executive Summary:

We would like to ask the Board to approve the request to apply for the California Department Public Health/California Home Visitation Program grant funding for Public Health's Home Visitation Program. The amount of $470,000 will be awarded each year for the next 5 years.

This program is critical to Lake County's pregnant and parenting individuals. This program promotes improved pregnancy outcomes, nurturing parenting skill, positive child health and development, and family self-sufficiency.

This grant is anticipated to fund July 1, 2023, through June 30, 2028, in the amount of $470,000 each fiscal year.
