Subject: Approve Health Care Program for Children in Foster Care Certification Statement and Authorize the Board Chair to Sign

Executive Summary:

On December 9, 2025, your board approved Resolution No. 2025-136 allowing Lake County Health Services to apply for funding in the amount of $465,591 to operate the Health Care Program for Children in Foster Care (HCPCFC) for FY 25/26. The HCPCFC program provides public health nurse expertise in meeting the medical, dental, mental and developmental needs of children and youth in foster care and is the foundation of meeting state mandates and the maintenance of essential services countywide.

We now request that your board approve and sign the adjoining Health Care Program for Children in Foster Care Certification Statement.
