Subject: Approve Amendment No. One to the Agreement between County of Lake and Clover Valley Guest Home for Adult Residential Support Services and Specialty Mental Health Services for Fiscal Years 2022-2025 in the Amount of $550,000.00 and Authorize the Board Chair to Sign the Agreement.

Executive Summary:

Utilization of this service has increased significantly, as Lake County Behavioral Health Services Department has transitioned more beneficiaries to a lower level of care. There are currently six (6) clients residing at Clover Valley. Four (4) of the six clients are on LPS conservatorship living at Clover Valley who were brought back in County from a higher level of care. The other two (2) clients had been experiencing increased symptoms that resulted in homelessness, hospitalizations, and risk of conservatorship.
