Subject: Approve the Agreement between County of Lake and Parkview Healthcare Center for Adult Residential and Skilled Nursing Support Services and Specialty Mental Health Services in the amount of $52,850.00 for fiscal year 2024-25 and Authorize the Board Chair to Sign.
Executive Summary:
Parkview Healthcare Center is a Medicare/Medi-Cal licensed Skilled Nursing Facility (SNF) in
Hayward, CA. The facility has 121 beds licensed for skilled nursing, 61 of which are in a secured
unit with delayed egress. The staff members at Parkview Healthcare Center receive specific Crisis Prevention and Intervention (CPI) training to better care for our patients with mental health issues. The facility hired a Medical Director focused on treating adults with mental health issues, along with a Program Coordinator who will oversee the secured unit. The facility also hired security staff to ensure the environment is as safe as possible, both inside and on the facility grounds.
Lake County Behavioral Health Services is continually exploring bed/placement options for our members needing this level of care. We conducted an Request for Qualifications process with this provider.
