Subject: Consideration of an Additional $15,000 Transfer in Lump Sum from General Fund to Public Guardian Account for "BB", to be Used by Public Guardian for Monthly Patch Payments Not To Exceed $5,000 per Month for his Placement at Nazareth Classic Care, a Private-Pay Facility That Does Not Accept Medi-Cal or Medicare

Executive Summary:
On October 17, 2024, we brought to your board a request for patch payment for Public Guardian conservatee, "BB", from general funds in the amount of $30,000 to cover six (6) months of placement, which your board approved, as there was no alternative funding source. Although BB had Medi-Cal coverage, the only facility that would accept BB, Nazareth Classic Care, did not accept Medi-Cal. Had funding not been secured, BB would have been released to the street. BB is conserved, and unable to live independently given his medical condition, even with IHSS support. BB is a 290 registered sex offender. BB was being held in Lake County Jail, had served his sentence, and was ready for release. The jail could not continue to hold him, he had served his time. Your board requested quarterly updates on our progress to move BB out of private pay into a Medi-Cal covered facility.

On November 1, 2024, BB was placed in Nazareth Classic Care.

On February 11, 2025, we brought your board our 3-month update. We reported BB had been doing well in his current placement at Nazareth Classic Care, and his immediate medical issues had been addressed, both of which we believed would make his placement in a Medi-Cal covered facility easier. We reported that we had identified numerous facilities throughout the state of California, a portion of which were suitable for BB. We were contacting these facilities, repeatedly, to seek placement for BB. One facility had placed BB on their wait...
