SUBJECT: Approve Amendment 1 to the Agreement between the County of Lake and Paragons, LLC. For the Coordination of Activities Involved with the No Place Like Home (NPLH) Grant for Fiscal Year 2018-19.
EXECUTIVE SUMMARY: Attached, for your approval, is Amendment 1 to the Agreement between the County of Lake and Paragons, LLC. For the Coordination of Activities Involved with the No Place Like Home (NPLH) Grant for Fiscal Year 2018-19. As Lake County Behavioral Health Services (LCBHS) has been working closely with Paragons, LLC. on activities associated with the NPLH grant, LCBHS would like to continue to move forward with Paragons, LLC.
FISCAL IMPACT: __ None _X_Budgeted __Non-Budgeted
Original Contract: $10,000
Amendment 1 Increase: $55,250
Total New Contract: $65,250
FISCAL IMPACT (Narrative): The amount budgeted for the NPLH technical assistance grant is $75,000 of which $55,250 has not yet been allocated; therefore, LCBHS is requesting approval of Amendment 1 to the Agreement between County of Lake and Paragons, LLC. For Fiscal Year 2018-19 for an increase of $55,250 for a new contract maximum of $65,250.
