Subject: Approve Agreement Between the County of Lake and Catholic Charities of Northwest California for Homelessness Prevention and Diversion Services, in an Amount Not to Exceed $400,000, for the Term Commencing Upon Issuance of a Letter of Intent Authorizing Services Through June 30, 2027 and authorize the Chair to sign the agreement

Executive Summary:

The Board is requested to consider approval of Agreement with Catholic Charities of Northwest California to provide Homelessness Prevention and Diversion services in Lake County using Homeless Housing, Assistance and Prevention (HHAP) Round 3 and Housing and Homelessness Incentive Program (HHIP) funding. The program is designed to stabilize households at imminent risk of homelessness and to divert households from entering the homeless response system through targeted financial assistance, housing problem-solving, mediation, and short-term case management.

Catholic Charities was selected through a competitive Request for Proposals issued by the Lake County Continuum of Care (LCCoC) and Lake County Behavioral Health Services. The contractor demonstrated experience delivering Housing First-aligned prevention and diversion services, administering flexible financial assistance, and coordinating with Coordinated Entry System (CES) partners.

The agreement authorizes services through June 30, 2027, with total compensation not to exceed $400,000. Funding is fully supported by HHAP and HHIP sources, with no impact to the County General Fund.
