Subject: Approve Agreement Between County of Lake-Lake County Behavioral Health Services and as Lead Agency for the Lake County Continuum of Care and Adventist Health Clear Lake Hospital, Inc. In the Amount of $184,000 for the Period June 1, 2025, Through October 31, 2026 and authorize the Chair to sign.

Executive Summary:

Lake County Behavioral Health Services (LCBHS), as the lead agency for the Lake County Continuum of Care (LCCoC), oversees a Whole Person Care Grant initiative to improve coordinated care, including homelessness outreach and prevention services. Following a prior Request for Proposals, Adventist Health Clear Lake was selected as a qualified nonprofit provider and contracted to deliver services supporting the Lake County Pathways HUB Model and LCCoC's Coordinated Entry System.

This agreement formalizes the continuation of services from June 1, 2025, through October 31, 2026, with compensation not to exceed $184,000.
