Subject:                     Approve Amendment No. One to the Agreement between County of Lake and the SmithWaters Group for Patient Rights Advocacy Services for Fiscal Years 2022-2025 in the Amount of $135,000.00 and authorize the Board Chair to sign.

 

Executive Summary:

 

Lake County Behavioral Health Services entered into an Agreement with the SmithWaters Group on July 1, 2022 for a three-year contract term. The original Agreement only specified funding for the first year of services. This Amendment clarifies the funding for the entirety of the contract period.
