Subject: Consideration of Approval of Agreement Between County of Lake -Lake County Behavioral Health Services as Lead Agency for the Lake County Continuum of Care and Lake County Office of Education for Fiscal Year 2024-27 in the Amount of $125,000.00
Executive Summary:
The Housing First model prioritizes securing stable housing as the essential first step for unaccompanied youth experiencing homelessness. This program provides immediate emergency assistance, rapid rehousing, and temporary shelter with a focus on high-risk youth. In addition to housing support, the program offers comprehensive services including case management, mental health counseling, substance use treatment, and life skills training aimed at fostering long-term stability.
Youth will also be connected to critical resources such as education, job training, and medical services, and assisted with securing permanent housing and rental assistance. To assess program effectiveness, data will be collected on key outcomes including successful housing placements, client satisfaction, and improvements in education and employment. All materials will be designed to be accessible and culturally sensitive, ensuring they meet the unique needs of the youth we serve.
