Subject: Consideration of the Agreement Between County of Lake Continuum of Care and Adventist Health Clear Lake Hospital Inc. to Provide Services Under the Homeless Housing Assistance and Prevention (HHAP) Youth Set- Aside Initiative in the Amount of $127,665.52 for the Period of December 1, 2024, to June 30, 2027.

Executive Summary:
Lake County Behavioral Health Services (LCBHS), acting as the lead agency for the Lake County Continuum of Care (LCCoC), is entering into an agreement with Adventist Health Clearlake Hospital, Inc. to expand services under the Homeless Housing, Assistance and Prevention (HHAP) Youth Set-Aside initiative. This agreement supports the Hope Center's efforts to provide Rapid Rehousing and support services tailored to unaccompanied Transitional Age Youth (TAY) experiencing homelessness in Lake County.

Under the agreement, two of the Hope Center's 21 transitional housing beds will be dedicated to TAY, with a goal of serving at least eight youths by June 30, 2027. Participants will receive shelter, meals, individualized care plans, and access to comprehensive services including healthcare, behavioral health referrals, education and employment assistance, and family reunification support.

This initiative aligns with LCCoC's broader strategies, including integration with the Homeless Management Information System (HMIS) and Coordinated Entry System, and is part of a countywide effort to reduce homelessness and improve outcomes for vulnerable youth.

The total contract amount shall not exceed $127,665.52, and the agreement will be effective from December 1, 2024, through June 30, 2027.
