Executive Summary:

This Amendment No. 2 represents a three-month extension of services through October 24th, 2023. Lake County Behavioral Health Services Department, as the lead Administrative Entity of the Lake County Continuum of Care, issued a Request for Proposals for Temporary Emergency Shelter Services. The RFP was not successful, in that no applicant was selected. The Lake County Continuum of Care Executive Committee voted to extend sheltering services provided by Sunrise Special Services Foundation for three months to allow for another RFP to be conducted. These services are funded by the Homeless Housing, Assistance and Prevention program.
