Subject: Approve Data Sharing Agreement between Partnership Healthplan of California and County of Lake, Behavioral Health Services and authorize the Behavioral Health Director to sign

Executive Summary:

Lake County Behavioral Health Services, as the Administrative Entity for the Lake County Continuum of Care (LCCoC), desire to enter into a data sharing agreement with Partnership Healthplan of California (PHP). The goal of this data sharing agreement is to coordinate care between the LCCoC and PHP by allowing exchange of data, including Homeless Management Information System (HMIS) data.

Execution of this Agreement also allows PHP to release funding to the LCCoC in the following amounts (by calendar year):

2022 - $187,832
2023 - $1,502,661
2024 - $2,066,159
