Subject: Ratify California Home Visitation State General Fund Evidence-Based Home Visiting (CHVP SGF EBHV) Funding in the Amount of $470,413 Annually for Fiscal Years 23-24 Through 27-28.
Executive Summary:
The CHVP SGF EBHV program expands our Maternal, Child, and Adolescent Health program, which allows our staff to provide evidence-based support for pregnant mothers, new infants, and their families, in partnership with Healthy Families of America. This program promotes improved pregnancy outcomes, nurturing parenting skills, positive child health and development, and family self-sufficiency by providing home-visiting services to populations that may benefit from the innovative approach outlined by Healthy Families of America.
We hope to increase awareness of available services and promote positive outcomes and family successes. To do this, Community Health Workers, a Public Health Nurse, and a Community Health Nurse will offer in-home visits to underserved and Medi-Cal-covered families to provide connection to community resources, education on health issues and parenting skills, and act as advocates.
This grant award is $470,413 annually, from July 1, 2023, through June 30, 2028.
Our request to apply for this funding was approved by your board on April 25, 2023.
