Memorandum

Date: October 27, 2026

To: The Honorable Lake County Board of Supervisors

From: Lisa Faraco, Health Services Director

Subject: Approve Fourth Amendment to the Agreement for Medical Services in Lake County Detention Facility by and Between California Forensic Medical Group, Inc. (CFMG) and the County of Lake to Incorporate CalAIM (California Advancing and Innovating Medi-Cal) Justice-Involved Initiative and Medication-Assisted Treatment (MAT) services, effective October 1, 2026, and authorize the Chair to Sign

Executive Summary:

As your Board is aware, California Forensic Medical Group (CFMG) is the contractor providing medical services at the Lake County Detention Facility. The County and CFMG have agreed to amend the Agreement to incorporate state required services related to the CalAIM (California Advancing and Innovating Medi-Cal) Justice-Involved Initiative, including Medication-Assisted Treatment (MAT) and Medi-Cal billing support.

The amendment maintains the existing base annual compensation of $4,577,574.04 for January 1, 2026, through December 31, 2026, and establishes additional compensation of $541,926 annually for 3.10 full-time equivalent (FTE) positions providing CalAIM and MAT coverage and MAT services for up to 25 daily program participants.

Additionally, the amendment replaces Exhibit A with an updated scope of work outlining CFMG’s responsibilities for medical and behavioral health care coordination, discharge planning, substance use disorder treatment, and CalAIM billing support.

Finally, the amendment replaces the staffing matrix and revises Section 24 to require prior written County approval for subcontracting, while clarifying CFMG’s responsibility for subcontractor performance and compliance.
