Subject: Consideration of Agreement Between the County of Lake, Acting Through Lake County Behavioral Health Services as Lead Agency for the Lake County Continuum of Care, and North Coast Opportunities, Inc. for Rapid Rehousing Services for Fiscal Years 2025-26 and 2026-27.
Executive Summary:
The Lake County Board of Supervisors is requested to consider an Agreement between the County of Lake, through Lake County Behavioral Health Services as the Lead Agency for the Lake County Continuum of Care (LCCoC), and North Coast Opportunities, Inc. (NCO) for the provision of Rapid Rehousing services for Fiscal Years 2025-26 and 2026-27.
Following a competitive Request for Proposals process, NCO was selected as the qualified provider to deliver Housing First, low-barrier rapid rehousing services to individuals and families experiencing homelessness in Lake County. Services include housing search and placement, rental and utility assistance, housing stabilization, landlord engagement, and case management supports designed to help households quickly obtain and maintain permanent housing.
The agreement term is for 27 months, through June 30, 2027, with a total contract amount not to exceed $527,625.96, funded through the various Homeless Housing, Assistance and Prevention (HHAP) grants and Housing and Homelessness Incentive Program (HHIP) grant administered by the County on behalf of the COC. Over the grant period, the program is expected to serve at least 40 participants, with performance measures aligned to HUD and Continuum of Care requirements, including permanent housing exits, income increases, and reductions in returns to homelessness.
