EXECUTIVE SUMMARY:  Attached, for your approval, is the First Amendment to the Agreement between the County of Lake and Manzanita House for Adult Residential Support Services and Specialty Mental Health Services for Fiscal Year 2016-17.

BACKGROUND AND DISCUSSION:  Located in Nice, Ca. Manzanita House is one of two Board and Care Facilities within the County of Lake that serve adults.  As they are in County, clients placed at Manzanita House are actively engaged in Lake County Behavioral Health Services and can participate in their Recovery Program leading to positive outcomes and a successful return to the community.  Manzanita House has proven to be a cost-efficient alternative for care to clients who are ready to be transitioned from Institutes for Mental Disease (IMDs) or have been brought back to the county from out of county placements.

Lake County Behavioral Health has identified that  utilization will not reach the original contract maximum; therefore, Lake County Behavioral Health is requesting a decrease in the total maximum payable under the Agreement of $14,000 for a new contract maximum of $106,450.
