SUBJECT: Approve the First Amendment to the Agreement between the County of Lake and Clover Valley Guest Home for Adult Residential Support Services and Specialty Mental Health Services for Fiscal Year 2016-17 for the amount of $59,760; and authorize the Chair to sign.
EXECUTIVE SUMMARY: Attached, for your approval, is the First Amendment to the Agreement between the County of Lake and Clover Valley Guest Home for Adult Residential Support Services and Specialty Mental Health Services for Fiscal Year 2016-17.
BACKGROUND AND DISCUSSION: Clover Valley Guest Home is a Board and Care Facility specific to the needs of older adult clients requiring placement. Clients placed at Clover Valley Guest Home are actively engaged in Lake County Behavioral Health services and can participate in their Recovery Program leading to positive outcomes and a successful return to the community. Clover Valley Guest Home has proven to be a cost-efficient alternative for care to clients ready to be transitioned from Institutes for Mental Disease (IMDs).
As an additional Lake County Medi-Cal beneficiary has been placed at Clover Valley, Lake County Behavioral Health would like to increase in the total maximum payable under the Agreement by $19,610 for a new contract maximum of $59,760.
