SUBJECT: Adopt Resolution Approving the Evergreen Medi-Cal Targeted Case Management (TCM) Provider Participation Agreement (PPA) Between the County of Lake and the California Department of Health Care Services and Authorize the Director of Health Services to Sign.
EXECUTIVE SUMMARY:
The Health Services Department recently received notification from the California Department of Health Care Services that the TCM Program is transitioning to an evergreen Provider Participation Agreement effective July 1, 2017. This transition will allow the Agreement to be continuous for the Local Government Agency’s term of participation in the TCM Program.
The goal of the TCM Program is to provide Targeted Case Management (TCM) services to eligible Medi-Cal beneficiaries. It is the intent to assist eligible individuals in gaining access to needed medical, social, educational, and other services.
