EXECUTIVE SUMMARY: Attached, for your approval, is the First Amendment to the Agreement between the County of Lake and Davis Guest Home for Adult Residential Support Services for Fiscal Year 2016-17.
BACKGROUND AND DISCUSSION: Davis Guest Home, which is located in Modesto, is an Adult Residential Living Facility licensed by the State of California. The Board and Care Facility is specific to the needs of adult clients that require placement that provides a structured environment due to the mental health challenges. The structured environment offers a non-restrictive, supervised program which focuses on each resident’s specific needs and interests. Davis Guest Home has proven to be a cost-efficient alternative for care to clients who need this type of structured environment. Lake County Behavioral Health (LCBH) pays a daily patch rate of $98 per day which is our cost after the client’s portion is paid.
As additional Lake County Medi-Cal beneficiaries have been placed at Davis Guest Home, LCBH is requesting an increase of $18,424 for a new contract maximum of $54,194.

FISCAL IMPACT (Narrative): LCBH is requesting approval of the First Amendment to the Agreement with Davis Guest Home for an increase of $18,424 for a new contract maximum of $54,194. The funds for this increase have been re-allocated from unassigned funds within the same level of care.
