SUBJECT: Consideration of Contract Change Order No. 4 for Foard Road at Anderson Creek Bridge Replacement Project, Federal Project No. BRLO-5914 (068); Bid No. 16-03
EXECUTIVE SUMMARY: Attached for the Board’s review and approval is Contract Change Order (CCO) 4 for the subject project.
Contract Change Order No. 4 addresses the settlement of four (4) Potential Claims for the subject project.
This change resulted in an increase of $47,000.00 to the current contract amount of $730,994.09.
This project is fully funded by the Federal Highway Administration (FHWA) thru Federal Highway Bridge Program (HBP) funds and California Toll Credit funds. Therefore, this project is 100% federally and state funded thru construction.
Staff recommends approval by the Board of Supervisors of CCO 4 and that the Board authorize the Chair to execute said CCO for an increase of $47,000.00 and a revised contract amount of $777,994.09.

RECOMMENDED ACTION: Staff recommends approval of Contract Change Order No. 4 for an increase of $47,000.00 and a revised contract amount of $777,994.09; and authorize the Chair to execute said Contract Change Order.
