SUBJECT: Adopt Resolution to approve the Standard Agreement between the County of Lake and the Department of Health Care Services for Fiscal Year 2017-18 through Fiscal Year 2020-21 and to authorize the Behavioral Health Administrator to sign the Standard Agreement.
EXECUTIVE SUMMARY: Attached, for your approval is the Standard Agreement between the County of Lake and the Department of Health Care Services for the Strategic Prevention Framework (SPF) Partnerships For Success (PFS) project. This Standard Agreement outlines and discusses the program specifications including scope of work, assessment and data collection tools, funds provision, general terms and conditions, special terms and conditions, additional provisions including HIPAA Business Associate Addendum, travel reimbursement information, and contractor’s release.

Total Contract Amount: $400,000
Year 1 (FY17-18): $100,000
Year 2 (FY18-19): $100,000
Year 3 (FY19-20): $100,000
Year 4 (FY20-21): $100,000
FISCAL IMPACT (Narrative): The contract maximum for this Standard Agreement is $400,000 with $100,000 to be allocated per each Fiscal Year starting with the period of July 1, 2017 through June 30, 2021.
