SUBJECT: Adopt Resolution Authorizing the Lake County Health Services Department to Submit an Application for the California
Department of Public Health's (CDPH), Oral Health Program
(OHP) for Fiscal Years 2017 through 2022 Not to Exceed an
Amount of $820,250

EXECUTIVE SUMMARY:
The Health Services Department has received notification from the California Department of Public Health (CDPH), Oral Health Program (OHP) of available funding through the recently passed Proposition 56 in the amount of $164,050 annually for Fiscal Years 2017-2022.
This grant funding has the following four focus areas:
1. To improve oral health by addressing determinants of health and promoting healthy habits and population-based prevention interventions to attain healthier status in healthy communities.
2. Align dental health care delivery system, payment systems, and community programs to support and sustain community-clinical linkages for increasing utilization of dental services.
3. Collaborate with payers, public health programs, health care systems, foundations, professional organizations, and educational institutions to expand infrastructure, capacity and payment systems for supporting prevention and early treatment services.
4. Develop and implement communication strategies to inform and educate the public, dental teams, and decision makers about oral health information, programs, and policies.
Health services will be using the funding for support planning, disease prevention, surveillance, education and linkage to treatment programs.
