SUBJECT: Adopt the Resolution approving the Standard Agreement for the Substance Abuse Block Grant (SABG) Multi-Year Agreement between the County of Lake and Department of Health Care Services for Fiscal Year 2017-18 through Fiscal Year 2019-20 and authorize the Behavioral Health Administrator to Sign the Standard Agreement.
EXECUTIVE SUMMARY:
This is the Standard Agreement for the Substance Abuse Block Grant Multi-Year Agreement between County of Lake and DHCS which funds the provision of providing alcohol and other drug prevention, treatment, and recovery support services. The new contract amount for the three year period through 2020 represents an increase of $96,195 over the prior three year term.
This Multi-Year Agreement outlines and discusses the Scope of Work, Program Specifications, Budget Detail and Payment Provisions, Funding, Special Terms and Conditions, Additional Provisions, Privacy and Information Security Provisions, and Information Exchange Agreement between the Social Security Administration and the California Department of Health Care Services.
