SUBJECT: Approve Amendment 1 to the Agreement between the County of Lake and Clover Valley Guest Home for Adult Residential Support Services and Specialty Mental Health Services for Fiscal Year 2017-18 for the amount of $69,300 and authorize the Chair to sign the Amendment.
EXECUTIVE SUMMARY: Attached, for your approval, is Amendment 1 to the Agreement between the County of Lake and Clover Valley Guest Home for Adult Residential Support Services and Specialty Mental Health Services for Fiscal Year 2017-18.
BACKGROUND AND DISCUSSION: Clover Valley Guest Home is a Board and Care Facility specific to the needs of older adult clients requiring placement. Clients placed at Clover Valley Guest Home are actively engaged in Lake County Behavioral Health services and can participate in their Recovery Program leading to positive outcomes and a successful return to the community. Clover Valley Guest Home has proven to be a cost-efficient alternative for care to clients ready to be transitioned from Institutes for Mental Disease (IMDs).
As placements have changed at Clover Valley, Lake County Behavioral Health is requesting to decrease the total maximum payable under the Agreement by $11,000 for a new contract maximum of $69,300.

FISCAL IMPACT (Narrative): Lake County Behavioral Health is requesting approval of the Amendment 1 to the Agreement for a decrease in the total maximum payable under the Agreement in the amount of $11,000 for a new contract maximum of $69,300. The funds from this decrease will be re-allocated to other contracts within the same level of care.
