SUBJECT: Approve Contract Between Lake County Department of Social Services and Lake County Office of Education in the Amount of $45,000.00 for FY 2018-2019 for Case Management Services for Children in Out of Home Care; and authorize the Chair to sign
EXECUTIVE SUMMARY:
For many years the Lake County Department of Social Services and Lake County Office of Education have coordinated to provided Case Management Services to assist foster youth, including minimizing changes in school placement, enhancing educational prospects, facilitating communication between all parties involved, and helping cater to youth with special education needs. Social Services works in partnership with Office of Education to achieve identified goals primarily by using joint problem solving and working together to implement a process designed to share information and data efficiently.

RECOMMENDED ACTION:
Approve Contract renewal Between the County of Lake and Lake County Office of Education in the Amount of $45,000 from July 1, 2018 to June 30, 2019 for case management services of children living in out of home care and authorize the Chair to sign.
