EXECUTIVE SUMMARY:

On September 27, 2011, the Department of Public Works entered into an Agreement with Quincy Engineering, Inc. for Engineering Services for Four (4) Bridge Replacement Projects and Two (2) Bridge Rehabilitation Projects in Lake County, CA. The Agreement was modified by Modification One and Amendments One thru Seven to allow for the completion of extra work which was required during the design phase and to extend the term of the Agreement.
Cast-in-drilled-hole (CIDH) piles were installed by the contractor at Robinson Creek Bridge at Mockingbird Lane. Gamma-Gamma Logging (GGL) testing, required to be performed prior to the acceptance of the CIDH piles, detected anomalies in a few piles. Cross-hole Sonic Logging (CSL) testing was then performed on the anomalous piles to determine the extent of the anomalies. Based on the results of the CSL testing, Quincy and its geotechnical sub-consultant performed detailed analyses in order to confirm that each anomalous piles indeed has the required capacity.
This amendment will provide for the additional effort required for Task 10, "Construction Support", in order to analyze the anomalous piles. Caltrans has concurred with this amendment and associated costs are eligible for reimbursement. As a result of this amendment, the contract amount will be increased as indicated below by task and by bridge:
