SUBJECT: Approve Agreement between the County of Lake and Redwood
Community Services, INC. for the Provision of SB163 Wraparound
Services in the Amount of $864,000, from September 1, 2018 to June 30, 2021;
and Authorize the Chair to sign.
EXECUTIVE SUMMARY:
County of Lake has contracted with Redwood Community Services, INC. for many years
to provide SB163 Wraparound services to qualified families. Lake County Wraparound
Program participants referred by the Interagency Placement Committee will receive
services including comprehensive assessment, individualized service plans, and
assistance with accessing resources. Contractor will also be responsible for maintaining
statistics and data to monitor participants’ progress in the program. This data will be
presented to Lake County Department of Social Services through monthly, quarterly, and
bi-annually reports.
