SUBJECT:                     Adopt the Resolution Authorizing Amendment A01 to the Standard Agreement for the Multi-Year Contract between the County of Lake and the Department of Health Care Services for Substance Use Disorder (SUD) Services between July 1, 2017 through June 30, 2020 and Authorizing the Behavioral Health Services Administrator to sign the Amendment

EXECUTIVE SUMMARY:  Attached, for your approval, is Amendment A01 to the Standard Agreement for the Multi-Year Contract between the County of Lake and the Department of Health Care Services for SUD services for Fiscal Year 2017-18 through Fiscal Year 2019-20.  The purpose of this Amendment is to modify some of the original terms & conditions in the Standard Agreement, as detailed in the Resolution, and increase the funding for Fiscal Year 2018-19 and Fiscal Year 2019-2020.

FISCAL IMPACT (Narrative):  The Amendment A01 to the Standard Agreement between the County of Lake and Department of Health Care Services for SUD services is being increased by $100,260 to be split during Fiscal Year 2018-19 and Fiscal Year 2019-2020 for a new maximum payable under this Agreement of $519,888 over three (3) Fiscal Years 2017-2020.
