SUBJECT: Approve Amendment One between the County of Lake and Ford Street Project for Substance Use Disorder Residential and Detoxification Services for FY 2018-19 for an increase of $15,000, and authorize the Chair to sign
EXECUTIVE SUMMARY: Attached, for your approval, is Amendment One between the County of Lake and Ford Street Project for Fiscal Year 2018-19. Under the Agreement, Lake County Behavioral Health Services (LCBHS), is obligated to pay a rate of $135.00 per day for Detoxification Services and $135.00 per day for Residential Services.
BACKGROUND AND DISCUSSION: The Ford Street Project is an organization that has been serving clients since September 1974. Although Ford Street operates in three basic areas including housing/shelter, drug and alcohol treatment, and job training/development, LCBHS is contracting for drug and alcohol treatment only.

Original Contract Amount: $ 50,000
Amendment 1 Increase: $ 15,000
New Contract Amount: $ 65,000
FISCAL IMPACT (Narrative): LCBHS is requesting approval of Amendment 1 to the Agreement with Ford Street Project for Fiscal Year 2018-19 for an increase of $15,000 for a new contract maximum of $65,000.
