SUBJECT: Adopt Resolution to Approve the Amendment 01 to the Standard Agreement between the County of Lake and the Department of Health Care Services for Fiscal Year 2017-18 through Fiscal Year 2020-21 and to Authorize the Behavioral Health Services Administrator to sign the Amendment.
EXECUTIVE SUMMARY: Attached, for your approval is the Amendment 01 to the Standard Agreement between the County of Lake and the Department of Health Care Services for the Strategic Prevention Framework (SPF) Partnerships for Success (PFS) project. This Standard Agreement outlines and discusses the program specifications including scope of work, assessment and data collection tools, funds provision, general terms and conditions, special terms and conditions, additional provisions including HIPAA Business Associate Addendum, travel reimbursement information, and contractor’s release.
The parties desire to amend the standard agreement in order to implement a budgetary shift of funds from one line item to another in year 2 and 3 and incorporate the Contractor vs Sub-recipient provision to comply with federal oversight requirements.

RECOMMENDED ACTION: Adopt Resolution to Approve the Amendment 01 to the Standard Agreement between the County of Lake and the Department of Health Care Services for Fiscal Year 2017-18 through Fiscal Year 2020-21 and to authorize the Behavioral Health Services Administrator to sign the Amendment.
