EXECUTIVE SUMMARY:
WELLPATH formerly known as California Forensic Medical Group (CFMG), has contracted with Lake County for jail medical services since January 1, 1996.
This First Amendment to the Agreement was approved June 25, 2019 and extended the current Agreement until August 30, 2019, while final revisions and approvals by all parties are completed to the new Agreement for Fiscal Years 2019-2022. However, the First Amendment to the Agreement omitted the payment amounts under this extension effective July 1, 2019 through August 30, 2019.
