SUBJECT: Adopt Resolution Authorizing the Health Services Director to Sign a Five Year Agreement in the Amount of $50,015 for the California Department of Public Health's (CDPH) HIV/AIDS Surveillance Program for FY2019-2024
EXECUTIVE SUMMARY:
County of Lake Health Services Department has been awarded a five-year grant from the California Department of Health's HIV/AIDS Surveillance Program.

Program activities for this five-year period will focus on five goals:

1. To maintain infrastructure for HIV surveillance by establishing and maintaining HIV case surveillance in health, medical, public health, and social services settings.

2. Collect and submit accurate, complete, and timely HIV surveillance data to the Office of AIDS (OA) that meet all data requirements set forth by the OA and the Centers for Disease Control and Prevention (CDC),

3. To maintain data security and confidentiality by protecting patient privacy and confidentiality and ensure that protected health information is stored and disclosed only in a manner consistent with California and federal laws and regulations as well as OA policies and procedures

4. To conduct HIV surveillance activities in a manner consistent with administrative, fiscal, budgetary and program guidance from CDPH, OA and CDC that ensure program management and coordination, and

5. To collaborate with partners to respond to the HIV epidemic to facilitate sharing data and resources to support progress toward meeting California's Integrated Plan goals and objectives.
