SUBJECT: Adopt Resolution authorizing Lake County Behavioral Health Services to approve Funding Allocations for the Whole Person Care Pilot Program (WPC Program).

EXECUTIVE SUMMARY:
Attached, for your review and adoption is the Resolution authorizing Lake County Behavioral Health Services to approve Funding Allocations for the Whole Person Care Pilot Program (WPC Program). These funding allocations are designated to integrate care for a particularly vulnerable group of Medi-Cal beneficiaries who have been identified as high users of multiple systems and continue to have poor health outcomes. The goal of the WPC Program is the coordination of health, behavioral health, and social services, as applicable, in a patient-centered manner with the goals of improved beneficiary health and wellbeing through more efficient and effective use of resources.

Available funding allocations for eligible activities approved by the WPC Program for Lake County Behavioral Health Services total Five Hundred Fifty Five Thousand One Hundred Thirty Six Dollars and Seventy Six Cents ($555,136.76).
