Subject:                     Approve Agreement Amendment between the County of Lake Health Services Department and Sheila McCarthy, Occupational Therapist to increase total hours worked and total compensation for contracted Occupational Therapy Services in Support of the California Children’s Services Program (CCS) and the Medical Therapy Program (MTP), and authorize Board Chair to sign                     

 

Executive Summary: The Health Services Public Health Division has an existing Agreement in place with Sheila McCarthy, Occupational Therapist to provide services for the CCS and MTU Programs through fiscal year 2019-2020.

 

Due to program needs for additional tele-med clinics during the months
of May and June the attached Agreement Addendum requests that maximum hours be increased from 18 hours to 33 hours per month and the maximum compensation be increased to offset these additional hours, and authorize Board Chair to sign the Amendment.

 

The original Agreement has also been attached for your reference. 

 

Ms. McCarthy has a Bachelor of Science Degree in Occupational Therapy from San Jose State University and has been providing occupational therapy for the last forty-two years including work done specifically with CCS and pediatric patients.
