Subject: Adopt the Resolution Approving Amendment A02 to the Standard Agreement for the Multi-Year Contract between the County of Lake and Department of Health Care Services for Substance Abuse Block Grant (SABG) funds for the contract period between July 1, 2017 and June 30, 2020 and Authorizing the Behavioral Health Administrator to Sign the Amendment.

Executive Summary: Attached, for your approval, is the Resolution Approving Amendment A02 to the Standard Agreement for the Multi-Year Contract between the County of Lake and Department of Health Care Services for Substance Abuse Block Grant (SABG) funds for the contract period between July 1, 2017 and June 30, 2020 and authorizing the Behavioral Health Services Director to sign the Amendment. The purpose of this Amendment is to modify some of the original terms and conditions in the Standard Agreement as detailed in the Resolution, increase the funding for Fiscal Year 2019-20 up to Two Million Sixty Thousand Nine Hundred Eighty Six Dollars ($2,060,986.00).
